Berks Community Health Center Expands Chronic Disease Management with Model of Care Grant

Berks Community Health Center (BCHC) is proud to announce it has received the Models of Care grant from the National Association of Community Health Centers (NACHC) through funding from Johnson & Johnson.

With this funding, BCHC has expanded its self-management program for patients with uncontrolled diabetes. The program equips patients with a free glucometer and a mobile app that transmits daily readings directly to the BCHC clinical team. Patients are actively supported by a Chronic Care Nurse and receive coordinated care from their provider, nutritionist, clinical pharmacist, and specialists as needed.

This initiative builds upon the success of BCHC’s 2021 Self-Monitored Blood Pressure (SMBP) program, which improved blood pressure control in over 1,000 enrolled patients. The principles this program is rooted on – patient empowerment, access to tools, proactive monitoring – help patients take control of their health and work towards positive, healthy outcomes.

BCHC serves over 20,000 patients annually, more than 85% of whom identify as Hispanic/Latino and nearly 25% as Black/African American. This expanded model of care reflects its ongoing mission to eliminate health disparities and provide accessible, compassionate care throughout Berks County.
